Minnesota's 2027 rate story was about a number insurers hide. Indiana's is about a number they are not hiding at all.

I pulled every Indiana individual medical rate filing out of the public SERFF health plan binders, the same way I did for Minnesota. Every on-exchange plan, both years, plan ID by plan ID, all 17 rating areas. What the binders show is a market getting more expensive and smaller at the same time. The median on-exchange plan that survives into 2027 costs 20 percent more at age 21. Two carriers are leaving. One is arriving, and only in two rating areas.

A note on the number I am comparing

Every premium here is for a 21-year-old, non-tobacco. Age 21 is the anchor of the federal default age curve, where the age factor is exactly 1.000, so comparing age-21 rates isolates the price change from any shift in age. Multiply by your own age factor to get your number. Indiana, unlike Minnesota, does permit a tobacco surcharge, so these are the non-tobacco base rates.

In Indiana, the headline number is the real number

For Minnesota I spent most of a post on the gap between what an insurer files as its average increase and what actually happens to the plans it keeps. Medica filed 13 percent while its surviving plans moved 1.5 percent, because the filed average absorbs everyone crosswalked off a deleted plan onto a pricier one.

Indiana does not have that gap. Here is what each continuing insurer filed, next to the median change on the plans it is keeping.

They line up. Anthem filed 15.6 percent and its surviving plans move 16.1 percent at the median. Ambetter filed 25.2 and moved 22.1. UnitedHealthcare filed 32.4 and moved 29.4. The two measures agree within about three points for all three carriers, and that tells you something specific: Indiana's increases are not hiding inside crosswalks and deleted networks. They are just large, and they are landing on the plans people already hold. The statewide weighted average of the filings is 18.9 percent. My plan-level median is 20.2 percent. Those are close for the same reason.

UnitedHealthcare is the story nobody headlined

UnitedHealthcare filed the biggest average increase in the state, 32.4 percent, and its plans back it up. In the Indianapolis area a UHC silver plan goes from about $490 to $620. In Bloomington a UHC bronze plan jumps 40.5 percent, the single largest increase on any surviving plan in Indiana.

And yet UnitedHealthcare is expanding. In 2026 it sold on-exchange plans in just three rating areas, all in central Indiana. For 2027 it files in ten. It is raising prices hard and moving into more of the state at the same time, which is what a carrier does when it decides the remaining risk pool is worth having at the new price.

Two carriers are leaving

CareSource is gone. It covered roughly 60,000 Indiana enrollees and sold in all 17 rating areas, a wider footprint than anyone. For 2027 there is no CareSource binder, and its plans, 374 plan and rating-area combinations, simply end. This is the single biggest change in the Indiana market, and it is why all 17 rating areas lose a carrier.

Cigna is gone too. Cigna is exiting the ACA market nationally for 2027, and Indiana is part of that. It was smaller here, fewer than 7,000 enrollees across six rating areas, but its exit stacks on top of CareSource's where the two overlapped.

The backfill that mostly is not there

Two carriers leaving would be easier to absorb if new capacity were arriving to replace them. Some is. Most is not.

UnitedHealthcare's expansion into seven new rating areas is the real backfill. Where UHC moves in, an area that lost CareSource holds its carrier count instead of dropping. That happens in four areas.

AmeriHealth Caritas is the genuinely new name, HIOS 11003, filing in Indiana for the first time. But its footprint is two rating areas, Indianapolis and Bloomington, twelve plans, and they do not undercut anyone. AmeriHealth's cheapest Indianapolis silver is $429.72, above the three cheapest Anthem silvers already on that shelf. A new entrant that prices at the top of the market did not come to start a price war.

Net it out and 13 of Indiana's 17 rating areas drop at least one carrier for 2027, counting by parent company. Southeastern Indiana around New Albany, across the river from Louisville, drops to a single on-exchange carrier: Ambetter, alone. In a one-carrier rating area the benefit design and the price are whatever that carrier decides, with nobody across the table.

Pick a rating area. Each row is one plan: where it priced in 2026, where it prices in 2027, and what moved. A plan with only a 2027 dot was not available in that area last year. A plan with only a 2026 dot will not be there next year, which for most of the muted 2026-only dots means CareSource.

A few things worth finding:

  • The increases cluster in the high teens and twenties. The middle half of surviving plans land between +14.9 and +25.9 percent. This is not a handful of outliers dragging an average.
  • The single worst increases are bronze and gold. A UHC bronze plan in Bloomington is up 40.5 percent, an Anthem gold in northwest Indiana up 40.0.
  • Ambetter has the only genuinely small increase. Its Elite Gold in the South Bend area went up 2.9 percent, the smallest in the state, though Ambetter's median is still north of 22 percent.

The same plan ID is not the same plan

Of the 426 continuing plan and rating-area combinations, 260 changed their marketing name while keeping the plan ID. A premium change on a renamed plan is not necessarily like-for-like, because the benefit design usually moved with the name. I flag every renamed plan in the tracker so you can see which comparisons deserve a second look.

How I built this, and what it does not tell you

Every number comes from the CMS Rates Table Template inside each insurer's SERFF health plan binder, the same standardized workbook in every state, joined to the plan names and metal levels on each binder's summary page. I took age 21, non-tobacco, on-exchange only.

Two specifics worth stating. Celtic Insurance Company, the Ambetter EPO/PPO entity, files in Indiana too, but every one of its plans is off-exchange, so none are here. Indiana's on-exchange Ambetter plans are the Coordinated Care HMO binder. And AmeriHealth's rate table is the only reason Indianapolis and Bloomington gain a new name at all.

The caveats:

  1. 2027 rates are proposed, not approved. The 2026 binders are closed and certified. The 2027 binders are still open, marked "Pending State Action" or "Submitted." Indiana's Department of Insurance reviews these and final rates can move.
  2. This is age 21, on-exchange, before subsidies. What you actually pay depends on your age, your income, and the premium tax credit, and with the enhanced subsidies expiring the gap between these numbers and what people pay is about to widen.
  3. Off-exchange plans are excluded. They are in the filings and in my data, but you cannot buy them on the exchange, so they are not here.
